Phoenix-N-Peace is committed through training and support to assist intellectual disabled, dually diagnosed adults,  and adolescents to attain individual goals in areas of communication, demonstrating appropriate behaviors and learning functional skills.

Phoenix-N-Peace, is based on the values that everyone deserves and needs to belong, to be loved, to be treated with dignity and respect, to be self -directing, and to be recognized for their inherent value as a unique person. Another guiding value is the right of persons with disabilities to be a part of the community, with the same opportunities, responsibilities and quality of life as available to all citizens. The overall goal of services is to promote acceptance in the normal life of the community in which the individual resides through social integration and participation in community opportunities. Support activities produce outcomes for the individual and skills are integrated to appropriate environments.  In addition, some of the Adult Day Support Programs provide special services such as health monitoring (e.g. monitoring of blood pressure) and certain types of medical care (e.g. basic wound care).  In addition, Phoenix-N-Peace provides many community services such as meals on wheels, gardening, and nursing home companion, and more.

Phoenix-N-Peace is a private agency-offering center & non-center based community support and a residential treatment facility for adult and adolescent individuals with physical disabilities with a primary diagnosis of intellectually disabled & mental illness.

Adult Day Support Programs are designed for adults (over 18 years of age) who are physically frail, intellectually impaired or living with a chronic illness. These day or evening programs have a double focus. They are intended to provide care and support for individuals daily so they can continue living in the community. In addition, they are intended to provide family caregivers with a much needed break from caregiving.  

Admissions Criteria


  • Individual must have significant limitations in adaptive skills in two or more of the following areas: communication, self-care, home living, social skills, community use, self-direction, functional academics, leisure, work, health and safety
  • Individual is not known to have tuberculosis in a communicable form
  • Individual is in a wellness state and does not need continuous nursing care
  • Individual does not have a history (within 2 years of creating unsafe conditions such as fire abuse
  • Individual’s active behavior Management Plan does not require the use of procedures, which would conflict with Phoenix-N-Peace, Inc. Human Rights Plan, which include time out, seclusion, intrusive aversive therapy, or mechanical restraints or a need for restrictive behavioral interventions.
  • The individual’s service needs are available by Phoenix-N-Peace, Inc.
  • The individual does not meet the characteristics described above
  • At least 18 years of Age
  • Primary Diagnosis of Intellectual disability (mental retardation)
  • At least 15-21 years of Age
  • Primary Diagnosis of Intellectual disability (mental retardation) with an IQ score below 70
  • Individuals may also have other disabilities such as blindness, paraplegia, deafness, mental illness, speech deficits, and cerebral palsy
  • Individual is not physically and verbally abusive to others


  • At least 18 years of Age
  • Diagnosis of Intellectual disability (mental retardation)
  • Determined eligibility for MR Home and Community Based Waiver by the Community Services Board
  • Day Support needs can be adequately met through services provided by the program
  • Express desire to participate in the program and willingness to participate in development and implementation of the Individual Service Plan
